Transitions, Inc. is a sober living operator and addiction treatment provider based in Northern Kentucky, running residential and outpatient programs across Kentucky, Ohio and Indiana. The organization moves people through every level of care itself, from intake and detox referral through long-term recovery housing.

A Multi-State Sober Living Operator

Transitions works across three states, not just one city, which is unusual for a recovery housing provider this size. A resident referred through a Kentucky court or a hospital in Cincinnati can end up in the same organization's care regardless of which side of the Ohio River the referral started on, and that reach shows up in how many insurers the organization is set up to bill.

One Organization Across the Continuum of Care

Transitions avoids handing a resident off between separate businesses, and instead runs its own intake and triage, residential rehabilitation, intensive outpatient and partial hospitalization programs, targeted case management, and long-term recovery housing under one roof. That structure means a case manager who intakes someone in early treatment can still be involved months later when that same person is living in one of the organization's recovery homes.

A Medication-Friendly Sober Living Operator

Transitions accepts residents on FDA-approved medication for opioid use disorder, including buprenorphine and methadone, across its housing programs. Many sober living operators still turn away residents on prescribed medication, and a policy that accommodates it broadens who the organization can actually serve without forcing a choice between housing and treatment.

Serving Kentucky, Ohio and Indiana

Transitions accepts Medicaid from all three states along with a long list of private insurers, which reflects how far its referral network reaches beyond Northern Kentucky itself. Kentucky law requires certification for recovery residences, and a family or provider working with Transitions should ask which of its housing programs currently holds certification through the Kentucky Recovery Housing Network, since that status is tracked program by program, not for the organization as a whole.
